I look forward to working closely with you to remove barriers, and build sustainable, healthy routines, & I believe the best outcomes come from meeting people where they are, listening first, and providing guidance that feels encouraging, accessible, and personalized.
What do care coordinator help with
Care coordinators are here to answer any questions you have about Bold. This can include what Bold is, why we’re different, and the potential cost. If you decide to move forward with a virtual visit with Bold’s care provider, your care coordinator will assist with scheduling and preparing for that visit. This includes discussing your goals, guiding you through your intake forms, and ensuring you’re comfortable using Zoom. We are here to help you!
