2. You discuss your health history, your goals, and the specific areas you want to feel better in
3. Your provider creates a personalized plan — not a generic checklist, but a healthy aging Care Plan tailored to you
4. You leave with concrete, actionable next steps across areas like sleep, energy, nutrition, or stress reduction.
