What’s the one thing you wish you could talk about more with your Primary Care Provider? A Bold healthy aging care appointment is a chance for you to go in-depth during a visit that’s oriented around you – your health, your habits, your goals. Your Bold Care provider is here to really listen to you. We’ll talk through how you’re feeling today and how you want to feel in the future. We’ll create a personalized healthy aging care plan that we can work through together.
Here's exactly what happens at a Bold Care appointment, from start to finish.
Before your appointment — no complicated prep required
All Bold Care appointments are virtual, so you can meet with a doctor from home — no travel, no waiting room, no fasting, no complicated forms. Once you confirm your healthcare coverage you’ll be able to book a provider appointment in a matter of days, not weeks. Here’s what you’ll do after signing up, but before your appointment:
- Complete a brief questionnaire. Understanding how you’re feeling today will help our doctors and nurse practitioners recommend the right things. This health questionnaire will allow your doctor to get to know you before your visit, so you can spend your appointment on the topics that really matter, not paperwork.
- Check your technology. You can meet with your provider over Zoom or a phone call, whatever feels best to you. If you selected Zoom, we’ll email and text you a link to join the Zoom before your appointment. If you chose a phone call, save our number in your contacts now so you recognize us when we call: (424) 577-5266.
- Think about your goals. This is optional, but helpful – take some time to “define your why”. Think about your Big Wins. Maybe it’s losing 20 pounds without medication, dancing at your granddaughter’s wedding, hiking The Rockies with your spouse in retirement, or finally figuring out why you’re wide awake at 3:00 AM. Whatever it is, no goal is too small for us to tackle. You don't need a formal list, just a sense of your priorities.

Who you'll meet: Your Bold Care provider
Bold Care providers are board-certified physicians and nurse practitioners who specialize in lifestyle medicine — a field focused on the root causes of how you feel, not just symptom management. Instead of a prescription for every problem, they look at how your daily habits (what you eat, how you sleep, how you move, how you manage stress, etc.) are affecting your health. Bold Care is a complement to your primary care provider, not a replacement. Think of it as the conversation your PCP doesn't have time to have.
The conversation may focus on:
- Weight and metabolic health: understanding how nutrition, movement, and daily habits affect your weight, and what to do about it.
- Condition and symptom management: Lifestyle strategies to better manage conditions like diabetes, hypertension, and arthritis and reduce their impact on your daily life.
- Restorative sleep: Reducing fatigue and insomnia without adding a new pill.
- Nutrition for vitality: Practical, low-inflammation food choices that boost energy.
- Movement & balance: Creating a plan to improve mobility and boost your balance (Bold members see a 46% reduction in falls).
- Stress & connection: Managing life transitions, anxiety, or "brain fog" through expert guidance.
Meet Dr. Desai
As a double board-certified physician in Internal Medicine and Obesity Medicine, I have over 20 years of clinical experience in primary care and preventive health. Together, we will collaborate to address and prevent age-related issues with a focus on healthy lifestyle management.
What to expect in our visits: We will discuss your medical history and your future health goals. Together, we will devise a plan to address current medical conditions, create health-promoting habits, and adopt positive lifestyle behaviors.

I am a Licensed Clinical Social Worker with extensive experience working with older adults. I use a trauma-informed and client-centered approach when working with clients on their healing journey.
What to expect in our visits: We will talk about any age-related problems, how they impact your physical and mental health, and the coping skills you can develop to address these issues.

I’m a Family Nurse Practitioner with over a decade of experience caring for older adults. My background as an ER nurse, Army combat medic, and internal medicine NP allows me to bring a practical, patient-centered approach to helping adults manage chronic conditions, maintain independence, and age with confidence.
What to expect in our visits: Our visits are collaborative, respectful, and focused on what matters most to you. I combine evidence-based medicine with realistic lifestyle strategies to support healthy aging, disease prevention, and long-term quality of life.

During your appointment — a minute-by-minute look
Here’s what will happen during your Bold Care appointment.
The first few minutes
Your provider will introduce themselves and orient you to the visit. You'll be asked to share what's been on your mind – what prompted you to book, and what you most want to address.
Your health history
This is a conversation, not a form. Your provider will ask about your current conditions, medications (bring a list if you have one, but it's not required), sleep, energy, activity, nutrition, and stress. You won't be rushed. You will be heard.
The real conversation
This is where Bold Care is different. Your provider isn't just reviewing labs or adjusting a prescription, they're exploring why you feel the way you do. Fatigue, weight changes, poor sleep, brain fog, aching joints: these aren't just symptoms to manage, they're signals your body is sending, and Bold Care providers are trained to listen to them.
Your care plan
Before the appointment ends, your provider will work with you on a personalized healthy aging care plan. Not a generic handout, this is a set of specific, realistic lifestyle changes tailored to your health, your goals, and your life. This might include changes to nutrition, sleep habits, movement, stress management, or a combination.

We don't order labs because we want to spend our time on the 75% of your health that happens outside of prescriptions.
For some health situations, we may not be the right provider for you. If you’ve recently received a cancer diagnosis, are on dialysis, or are experiencing certain other conditions, we may not be able to provide the level of care that you need. Our Care Coordinators will help talk through anything on your mind, and discuss different options available.
What’s next
Your provider will explain next steps and schedule your follow-up, as needed. The visit typically runs 45–60 minutes. After your appointment we’ll email you your personalized healthy aging care plan.

Bold Care isn't a one-time visit. Your care plan is a living document, and your follow-up appointments are where the real progress happens. Your provider will check in on what's working, adjust what isn't, and help you build on the changes you've already made. Most members notice meaningful differences (more energy, better sleep, less pain.) within the first few weeks of following their plan.

Q: Do I need to see my regular doctor before booking with Bold Care?
A: No referral needed. You can book directly.
Q: What if I'm not comfortable with video calls?
A: You can join by phone. All you need is the ability to make a phone call.

A: No, Bold Care will not share notes with your PCP, but you are welcome to transfer your records from Bold to your PCP’s office. You can upload any recent labs or other medical information into your Bold Care portal for your provider to reference.
Q: What if I need to reschedule?
A: We’ve reserved this hour just for you. If you can no longer make your scheduled appointment, please cancel it and free up the time for another patient to meet with a provider. Because our providers dedicate a full 45–60 minutes to each member, these appointments are highly requested and often book up quickly. If you can’t make it, please let us know so we can offer this window to another member waiting for care.
